Residential Care Manager - Carolina Youth Development Center

OVERALL JOB PURPOSE: The primary responsibility of the Residential Care Manager is to provide individual case management services to at risk youth residing in the Level 3 Group Homes. The Care Manager will administer assessments, coordinate treatment teams, create and monitor progress toward individual care plans, deliver interventions, facilitate programming and manage data.  The position operates under the supervision of the Lead Care Manager

Scope: Residential Case Manager Services include but are not limited to assessment, service planning, monitoring, coordinating and supervising family visits and family engagement, groups, community referrals, and educational/enrichment planning, coordination and support. Services are based on youth and residential staff needs.

This person is also expected to document weekly progress notes in CYDC’s data management system and share all information regarding the youth with the South Carolina Department of Social Services (SCDSS).

JOB RESPONSIBILITIES:

Case Management

Build Rapport: Establish a professional relationship and be a consistent role model to youth being served. Communicate and uphold Youth Rights. Promote effective communication between staff and youth.

Administer all assessments to identify needs and monitor progress of youth. Deliver Biopsychosocial and Level of Care Assessments, as well as the Comprehensive Clinical Summary, within the youth’s first 30 days of admission.

Help youth access basic needs such as clothing, dental/hygiene supplies, medical care, etc.  Assist with transportation services as needed.

Develop a comprehensive, Individual Care Plan for each youth that outlines education, personal development, independent living and family involvement goals within the youth’s first 30 days of admission.  Meet weekly with each individual youth for a minimum of 30 minutes per session to evaluate progress towards goals, adjust as needed, set new goals, track growth, make referrals and provide overall case management services in accordance with service standards. 

Deliver research and evidence based practices to support Individual Care plans. Develop and coordinate enrichment and education activities designed to build resiliency, job readiness or life skills through small groups, workshops and sessions. 

Refer and link youth to community based programs and services.  Serve as a liaison between youth and local employers as needed as youth build independent living skills.

Engage family members/caretakers as appropriate.  Participate in all Family Group Conferences and Foster Care Review Board Meetings. Supervise and track all Family visits.  Plan and coordinate opportunities for positive family engagement.

Serve as a school advocate to monitor grades, absences and school behavior, help youth set and achieve academic goals and adjust Care plans to ensure educational growth.

Participate in daily debrief and weekly treatment team meetings.

Document Progress:  Complete and submit all documentation in accordance with service standards.  Track weekly progress notes in CYDC’s data management system.

Review all youth charts on a bi-weekly basis and participate in Case File Reviews on a quarterly basis.

Administer semi-annual level of care evaluations for each youth.

Training/Ongoing Professional Development: 70%

Participates in internal meetings and minimally obtains required training such as agency Orientation, CPI, CPR/First Aid, Documentation, Medication Administration training, and other mandatory training, as directed by Human Resources or the Vice President of Residential Services. Actively seeks out training that builds skills in the assessment and service of the client population.  Provides one-on-one coaching with direct care staff as needed.

Teamwork, Quality Improvement, and Cultural Sensitivity: 20%

Promotes the spirit of teamwork. The Residential Care Manager will assist with crisis management, on-call problem solving and Prudent Parent decision making and shift coverage when needed. In addition, the Residential Care Manager will demonstrate the ability to be a good colleague and will strive to continuously improve the quality of the agency and self. These competencies will be carried out in order to increase the efficiency of the program.  The Residential Care Manager will accept cultural and individual differences, upholds the vision and mission of the agency and serve as an active participant in all agency initiatives to achieve these goals.

License or Certification:  Valid SC driver’s license with a good driving record.

Decision Making Authority:

Must possess the ability to make sound decisions weighing all of the factors for good judgment based on CYDC policies and procedures. Decisions must reflect good clinical skills as well as the best interests of the youth.

Maintain a caring relationship with youth while allowing for independent growth.

Demonstrate successful parenting with effective behavior management skills or have extensive experience in the child care field.

Be a positive role model to youth and staff

Exhibit organizational and writing ability to facilitate the maintenance of accurate and complete records.

Operate as a staff team player working together to achieve the goals of the program and of the agency.

Must possess the knowledge and skills necessary to work with a diverse population. Must have the skills to actively engage other professionals as well as excellent communication skills and the ability to develop well-rounded, behavioral interventions.

Education and Specific Training:

Minimum qualifications include: (1) Master's degree in social work, counseling, psychology or a related to social work preferred; Bachelor’s degree required plus two years of experience working with the population being served; or (2) Hold an active license in a field related to social work plus two years of case management experience in social work, counseling, psychology, education or a related field.

Experience:  Two years of social work experience working with the population being served.
